Client Requirements

The client from the United States wants to offer healthcare services in various specialties. The client wanted to develop a system to enhance their healthcare business by providing a value-added facility to patients. They are interested in developing Telehealth, Remote Patient Monitoring, and Chronic Care Management services.

The client wishes to develop an application through which specialists can securely connect providers and patients. Patients can quickly connect with healthcare providers online via audio and video calling. This allows for virtual visits and the monitoring of multiple medical conditions, including at least one chronic one.

It also assists in identifying eligible patients for the RPM and CCM programs. Patient engagement and assessment using virtual encounters, insurance verification, creating comprehensive telehealth, RPM, and CCM programs, and assigning them to patients.

Thinkitive Solution

Thinkitive's business analysts and subject matter experts starts the requirement analysis phase. During this phase, they worked on requirement gathering and competitive research. They then suggested a solution that best suited the client's business needs.

The BA team takes a call with clients on multiple occasions while creating detailed functional requirement documents. After this, we designed low fidelity prototypes and took timely approvals from the client. Thinkitive assembled a team of experts in both frontend and backend development. The innovative team employs a microservices based architecture and creates separate microservices for each module.

A suggestion was made to create a system with essential features. This system would enable providers to virtually connect with patients from any location at any time. The system would also have RPM and CCM modules, allowing the provider to find eligible patients and onboard them easily.

The patient, provider, and admin will each have their login, and the patient will take the vitals via the device. For more information, please see the diagram below.

Solution Highlights

  • 1. Telehealth on Demand

Telehealth enables remote access and management of healthcare services through digital information and communication. The highly secure system allows providers to connect patients instantly one on one via audio or video. The platform offers secure, safe video consultations with the providers. It can view patients' medical histories during the call for the best possible care.

The system uses HIPAA compliant messaging to communicate and share medical records via chat. The telehealth system helps to manage patient care. It can be used to conduct sessions, take vitals, record SOAP notes, order e-prescriptions, and generate billing for completed sessions.

  • 2. Customizable Remote Patient Monitoring Solution

  • Identify and Onboard eligible patients :

The system uses EHR data to identify patients with at least one chronic condition in the past six months. These patients are then invited to join the RPM program.

  • Customizable RPM Care Program :

The system enables providers to create customizable RPM programs based on the patients' health conditions. The system enables providers to create a library of RPM program templates and customize them before assigning them to patients.

  • Device Installation with only one click and Interaction with EHR :

The system enables providers to add RPM devices and assign them to patients. Devices help to gather data. This data includes weight, activity levels, blood pressure, heart rate, blood sugar, and other vital information. Healthcare providers can use this data to monitor progress.

  • Alerts and Notifications :

Alerts and notifications are sent to the provider and other users if the system receives vitals outside the normal range.

  • 3. Chronic Care Management for Chronic Condition Patients

  • Patients can use the system for several purposes. They can create personalized care plans that are focused on the patient. They can also track billable time and coordinate care services for caregivers and physicians.

The system enables providers to identify eligible patients with at least two chronic conditions. All CCM patients are included in the single tool.

Providers can connect patients 24 hours a day, seven days a week via audio or video call. The CCM system allows providers to create and assign customizable CCM programs to patients. It is straightforward to add a device to the CCM program. The system also enables providers to communicate with other patients via chat.

  • Customizable Dashboard :

The interactive dashboard tracks and monitors crucial healthcare metrics in one place. Users can delve deeper into the data and make well-informed, data-driven business decisions by utilizing various interactive features.

  • Patient Onboarding and Assessment :

Patients with two or more chronic conditions are eligible for the CCM program. Providers can pull patients' information from the EHR or manually enter it.

Patients and providers can sign consent forms, emphasizing the importance of doctors and patients working together. It allows us to make decisions based on exchanging relevant patient specific information. It also contains information on potential risks and benefits.

  • Create and Assign Care Plans :

Care plans can be created to effectively identify, manage, and treat patients' chronic conditions. Care plans help providers understand the patient's goals, objectives, and stage of illness. Depending on the patient's health, a provider may assign multiple plans to the same patient. Providers can add care team members and devices for patients according to their condition.

  • Monitor Patient 24*7 :

The system timely monitors the patient's vital signs and notifies the provider and others if any patient's health concerns. Patients can easily communicate with their respective providers in the event of a discussion or an emergency.

  • Easy Devices Installation and Integration with EHR :

The system makes it easy for the provider to identify and onboard the patients into the EHR system. Healthcare providers can collect patient health data. This includes weight, activity levels, blood pressure, heart rate, and blood sugar levels. They can track this information over time.
