The client from the United States wants to offer healthcare services in various specialties. The client wanted to develop a system to enhance their healthcare business by providing a value-added facility to patients. They are interested in developing Telehealth, Remote Patient Monitoring, and Chronic Care Management services.

The client wishes to develop an application through which specialists can securely connect providers and patients. Patients can quickly connect with healthcare providers online via audio and video calling. This allows for virtual visits and the monitoring of multiple medical conditions, including at least one chronic one.

It also assists in identifying eligible patients for the RPM and CCM programs. Patient engagement and assessment using virtual encounters, insurance verification, creating comprehensive telehealth, RPM, and CCM programs, and assigning them to patients.

The BA team takes a call with clients on multiple occasions while creating detailed functional requirement documents. After this, we designed low fidelity prototypes and took timely approvals from the client. Thinkitive assembled a team of experts in both frontend and backend development. The innovative team employs a microservices based architecture and creates separate microservices for each module.

A suggestion was made to create a system with essential features. This system would enable providers to virtually connect with patients from any location at any time. The system would also have RPM and CCM modules, allowing the provider to find eligible patients and onboard them easily.

The system uses HIPAA compliant messaging to communicate and share medical records via chat. The telehealth system helps to manage patient care. It can be used to conduct sessions, take vitals, record SOAP notes, order e-prescriptions, and generate billing for completed sessions.

  • Identify and Onboard eligible patients :

    The system uses EHR data to identify patients with at least one chronic condition in the past six months. These patients are then invited to join the RPM program.

    The system enables providers to create customizable RPM programs based on the patients' health conditions. The system enables providers to create a library of RPM program templates and customize them before assigning them to patients.

  • Device Installation with only one click and Interaction with EHR :

    Providers can connect patients 24 hours a day, seven days a week via audio or video call. The CCM system allows providers to create and assign customizable CCM programs to patients. It is straightforward to add a device to the CCM program. The system also enables providers to communicate with other patients via chat.

    The interactive dashboard tracks and monitors crucial healthcare metrics in one place. Users can delve deeper into the data and make well-informed, data-driven business decisions by utilizing various interactive features.
